Character's Name: Self Explanatory, First name Last Name (if known)
Preferred names/Nicknames: What the character prefers to be called and, or what others call him/her. You may leave blank if s/he has no nickname.
D.O.B.: Date of Birthday DD/MM/YYYY (Day/Month/year) May leave year and or day out if unknown.
Appearance: Description of the character's appearance. Must be as descriptive and through as possible. Two Paragraphs at least.
Personality: Description of the character's personality, including the way they think, how they see others, how they'd act in certain situations, e.t.c. Two paragraphs at least.
Short Bio/Life story:Short biography of the character. What events made the character who she or he is today? At least one sentence for each year of their life.
Interesting things: Some fun facts about the character. Could be something unexpected of them, favorite things, etc.
Powers/abilities: Start out with two abilities and two powers. It can be anything, just do not make it too extreme.
Strengths: Their stronger points.
Weaknesses: Areas to improve in/weaker points.
Favorite Quote: Self Explanatory
Picture (optional): If you want, you may find a picture that resembles your character, or draw your character. But since your appearance description is supposed to be as through as possible, it is not needed.
